Moving from Children to Adult Mental Health Service Survey

There are no right or wrong answers and you don’t have to answer any questions you don’t want to.
These questions are about your experiences of being supported by CAMHS (Children’s Mental Health Services) and AMHS (Adult Mental Health Services).
You will not be asked about any details of your mental health conditions or treatments.
Any information you disclose will be treated confidentially and if used in the report, as quotes or statistics, will only be referred to as, for example – 18 year old female (North Wales) etc.
